그 용도로만 쓰는 건 아니지만 smartmon이 있습니다. (hdparm으로 못보는 LVM 으로 설정된 디스크도 잘 보이네요)

# apt-get smartmontools

# smartctl -a /dev/sda

하면 됩니다.
